1. What types of breast augmentations are available?

Breast augmentation can be achieved through several different types of materials added to the breasts. With breast implants, the actual implant is inserted either above (less common) or below (more common) the breast muscle. In a natural breast augmentation, the patient’s own fat is injected into the fat layer in the skin.

Breast implants are flexible plastic shells that can either be filled with saline or silicone. Silicone gel implants always arrive sealed from the manufacturer. Silicone cohesive gel implants, also called “gummy bear implants”, have now been approved by the FDA and also arrive sealed. Saline implants can arrive either filled or unfilled.

The unfilled saline implants are rolled up and inserted through tiny tunnels created in endoscopic and “no scar” breast augmentations such as our transaxillary and TUBA methods. We are proud to use Mentor breast implants in our surgical procedures.

Fat transfer breast augmentation is virtually scarless and requires a liposuction procedure to harvest fat from the body. After being purified, the fat emulsion is injected into the breasts where about 75% of it remains permanently. It is for this reason that fat injections to the breasts will be over-injected by about 25% in the initial procedure.

2. Are breast implants safe – what if my breast implant ruptures?

Yes, silicone is just as safe as saline breast implants so long as they stay sealed. Older silicone implants should be replaced and patients should get checked for breast implant ruptures every year after the 10 year mark.

Saline implants can harbor bacteria, mold, and fungus if air is allowed to remain inside of the implant during surgery. This can make a patient sick when a saline breast implant ruptures. This is one of the many reasons why our surgical team is sure to remove every bit of air from the saline implants prior to sealing them up.

3. Can I have local anesthesia instead of general anesthesia?

Yes. Some of our patients can ask for local anesthesia instead of general. It is ultimately up to the surgeon to make the final choice of what type of anesthesia should be used. A fat transfer breast augmentation is almost always done using local anesthesia.

Since every patient is different, some of our patients may benefit from local anesthesia whereas others would do better with general. We use local anesthesia whenever possible because it is less of a risk to the patient’s health and is also more cost-effective (eliminates hospital fees and anesthesiologist fees). 4. What is a no scar breast augmentation?

Our no scar breast augmentation uses either implants or fat. For the breast implant procedure, this can be done through either the transaxillary (armpit incision with an endoscope) or TUBA (trans-umbilical breast augmentation) methods. Our team is specially trained in both of these procedures. Look at some of our breast augmentation photos to see his results.

A fat transfer to the breasts is scar-free with the exception of tiny punctures from the liposuction cannula used during the fat harvesting phase of the procedure. The incisions are so tiny, in fact, that no stitches are needed.

5. Where can breast augmentation incisions be placed?

Incisions for breast augmentation using implants can be placed in four different areas. The armpit (transaxillary), under the crease (inframmary), through the navel (TUBA) or along the border of the areola (areolar/periareolar). The transaxillary and TUBA methods are no scar incision placements and require specialized training to perform. Our team is fully trained in all four breast augmentation methods including the no scar techniques.

Fat transfer to the breasts uses injections of fat for augmentation of the breasts. There are no incisions made.

6. What shapes do breast implants come in?

Implants come in three different shapes: teardrop breast implants, standard profile (evenly round on all sides), and high profile (taller in height than width). These shapes are available in every size imaginable to accommodate a patient’s preference as well as body type.

7. How long do breast implants last?

The average lifespan for saline and silicone breast implants is about ten years. Though it isn’t necessary to automatically have breast implant replacement surgery when the ten-year mark arrives, it is a good idea to keep an eye on changes in breast shape and volume so that breast implant ruptures can be detected early. It is highly recommended to purchase a warranty from the implant manufacturer to protect against breast implant rupture at any time prior to the ten-year mark.

8. What if my breast implant ruptures?

Silicone implants, when ruptured, deflate at a slow pace whereas saline implants usually rupture suddenly or over a few hours. Silicone, if left to leak into the body, is unhealthy and could possibly be linked to serious health conditions. They should be removed immediately if leakage is detected.

Ruptured breast implants filled with saline can make a patient sick if air was left inside of the implant during surgery. This is because bacteria, mold, and fungus can grow inside of the implant and then be spread outside of the breast capsule upon rupture. These should also be removed immediately if leakage is detected.

Our team is careful to always remove every bit of air from saline implants prior to sealing them and closing up the patient. Patients can opt to purchase an extended protection plan for their implants to cover the cost of breast implant rupture during the first ten years. We perform both breast implant rupture removal and replacement in our breast augmentation centers in Commack, Long Island.

9. What if I’m not happy with my breast size after surgery?

Patients who are unhappy with their size after breast augmentation surgery are most often patients who made only a small upgrade in cup size. It is therefore a good idea to try on many different sizes, even those that you feel might be too large at first glance.

Many patients who try on larger-sized implants inside of their bra decide that they like the way the larger implants look versus the smaller size they originally had in mind. Bring in pictures of models or actresses who share your same body stature and weight but who have breast sizes that you would like to have.

Our team is very adept at seeing what cup size you have already and which implant size would bring you to the cup size you’re looking for.

10. What are the risks of breast implant surgery?

Though extremely rare, some serious complications can occur after having breast implant surgery. These risks include allergic reaction to or death from general anesthesia, post-surgical infection, capsular contracture in the breast requiring revision surgery, or rejection from the body of the implant (requiring immediate removal). 11. Where will my breast augmentation surgery be performed?

Surgery is usually performed in our AAAHC accredited breast augmentation New York surgical centers. For some patients, however, the procedure must be done in the hospital with an anesthesiologist present.
